Occupational Therapist OT Outpatient Therapies (PRN)

Empath Therapies a member of Empath Health is seeking a compassionate PRN Occupational Therapist (OT) to work with our team to deliver individualized care to patients in our outpatient clinic and in their homes throughout the Venice FL area.

Locations: Outpatient therapy clinic in Venice. Role also involves home health in Venice & surrounding areas.

Schedule: PRN role (mix of outpatient & home health visits)

What Youll Do

• Perform comprehensive patient assessments including OASIS assessments at required intervals
• Develop initiate and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes self-care techniques and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless holistic care

What Youll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment critical thinking and patient education skills
