Key Responsibilities:
Implement physician-approved care plans
Carry out physical therapy treatment plans established by the supervising Physical Therapist and physician delivering skilled interventions in the patients home.
Deliver direct therapeutic treatment
Administer therapeutic exercises manual therapy massage gait and balance training and physical modalities (heat cold ultrasound electrical stimulation) to relieve pain restore function and improve mobility.
Provide assistive device training
Instruct patients on the safe use of walkers crutches canes wheelchairs braces and prosthetics to support independent functioning at home.
Educate patients and families
Teach home exercise programs proper body mechanics and medical equipment safety to patients and caregivers to promote safe confident recovery between visits.
Monitor and document patient progress
Observe patients before during and after each session; record clinical findings; and submit timely progress notes to inform the supervising PT and care team.
Communicate changes in patient condition
Report any changes in a patients status or response to treatment to the Physical Therapist promptly recommending plan-of-care modifications as needed.
Coordinate interdisciplinary care
Participate in care conferences collaborate with agency personnel (nursing OT social work) and keep the team informed of patient progress to ensure seamless holistic care.
