JOBSUMMARY: 7a-3p M-F 7a-7p Baylor $27.25 - $31.75

Theprimary purpose of the LPN Wound Care Nurse position is to provide wound careprevention and treatment to residents and to assure resident safety under thedirection of a Registered Nurse attending physicians and/or Medical Directorand consistent with established wound care protocols.

JOBDUTIES & RESPONSIBILITIES:

Implementand maintain established wound care policies and protocols. Interpret these protocols to the supervisingRN physicians residents family members and others as appropriate.

Makerecommendations to the attending RN physician Medical Director DON and RegionalDirector of Clinical Operations concerning the wound care protocols preventivemeasures and specialty care of residents.

Participatein facility surveys made by authorized government agencies as requested by theDirector of Nursing.

Direct Care Functions

Conferwith RN Team Leaders to identify residents at risk for wounds or those whorequire assessment of wounds and treatment interventions.

Provideon-site assessment of residents to determine status and develop plan of care.

Implementpreventive measures and wound care based on resident assessment andestablished protocols.

Provideeducational health care services. Makerecommendations to the resident concerning nutrition hydration positioningprevention of skin breakdown management of chronic conditions etc.

Documentwound care assessments treatments rendered and communication with residentsfamilies and physicians in the electronic medical record.

Committee Functions

Participatein Interdisciplinary Care Plan Team to develop a comprehensive care plan forresidents.

Serveon the Quality Assurance Performance Improvement (QAPI) Committee to assess thefacility wound care status and improvement plan.

Attendsmorning clinical meetings.

Participatesin High Risk weekly meetings.

Personnel Functions

Cooperatewith other associates to achieve departmental objectives and maintain goodinterdepartmental and public relations.

Maintainan effective professional working relationship with physicians consultantsfamily members residents and associates.

Safety and Sanitation Functions

Assistin establishing an inventory of medical supplies and equipment to meet theneeds of residents.

Asappropriate use personal protective equipment and clothing when treatingresidents.

Reportoccupational exposures to blood body fluids infectious materials andhazardous chemicals according to facility policy and procedures


JOBREQUIREMENTS:

Education

Mustpossess a Nursing Degree from an accredited college or university or be agraduate of an approved LPN diploma program.

Experience

Musthave a minimum of one (1) year experience in a long-term care hospital homehealth or other health related organization.

Musthave previous experience in wound care or as a treatment management.

Certificationas a specialist in wound care management is preferred.
