Work Schedule: Monday - Friday 8:00 a.m. - 4:00 p.m.

What Youll Do

• Lead hospice admissions and care transitions within a hospital setting
• Provide advanced pain and symptom management for complex patients
• Deliver Hospice Houselevel care when inpatient placement isnt immediately available
• Seamlessly transition patients to home hospice care when Hospice House beds are full ensuring continuity comfort and dignity no matter the setting
• Partner closely with physicians and hospital care teams as a trusted clinical expert
• Support and educate patients families and clinicians on hospice philosophy and goals of care
• Advocate for comfort dignity and deeply patient-centered decisions

• Active RN license in New Hampshire (or eligibility)
• 2 years of RN experience (hospice palliative care ICU ED oncology or acute care preferred)
• Strong clinical judgment assessment and communication skills
• A compassionate presence and ability to support patients and families through complex emotional moments
