We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. This role is responsible for assessing planning implementing and evaluating individualized patient care plans while collaborating closely with physicians caregivers and interdisciplinary healthcare teams to ensure safe and effective outcomes.
The ideal candidate is organized clinically strong and passionate about delivering patient-centered care in a home health setting.
Responsibilities
Provide professional nursing care to patients in the home setting in accordance with physician orders organizational policies and regulatory standards.
Conduct comprehensive nursing assessments medication reconciliation and development of individualized care plans.
Monitor patient progress and update care plans based on changes in patient condition.
Ensure visit frequency and services align with physician orders and insurance authorizations.
Deliver and document patient and caregiver education using appropriate teaching strategies.
Perform assigned nursing treatments and procedures according to established standards of care.
Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and resource referrals.
Accurately document assessments interventions clinical decisions and patient progress within required timelines.
Communicate patient status updates and care plans during care transitions and staff handoffs.
Identify and report patient care concerns to leadership and physicians in a timely manner.
Supervise and support home health aide staff including completion of required supervisory visits and documentation.
Maintain productivity standards and complete assignments independently and efficiently.
Stay current on nursing best practices regulatory requirements and clinical guidelines.
Perform additional duties and projects as assigned.
Qualifications
Required
Associates Degree in Nursing or Diploma in Nursing
Active New Jersey Registered Nurse (RN) License
AHA Basic Life Support (BLS) Certification
Minimum of:
Six (6) months of RN experience OR
One (1) year of LPN experience
Previous experience in home health or hospice preferred
Basic computer proficiency and documentation skills
Ability to lift push and pull up to 50 lbs.
Ability to stand and remain active throughout the workday
Preferred
Bachelor of Science in Nursing (BSN)
