Type: local contract 40 hours per week 8am - 430pm
Job Summary: The Home Care Registered Nurse (RN) provides skilled nursing care to patients in their homes promoting independence comfort and optimal health outcomes. The RN works collaboratively with physicians caregivers and interdisciplinary teams to develop and implement individualized care plans.
Key Responsibilities:
Conduct comprehensive patient assessments including physical emotional and environmental needs
Develop implement and update individualized care plans
Administer medications treatments and wound care as prescribed
Monitor patient conditions and document changes accurately
Educate patients and families on disease management medications and home care techniques
Coordinate care with physicians therapists and other healthcare professionals
Ensure compliance with healthcare regulations and agency policies
Provide end-of-life care and support when applicable
Maintain accurate and timely clinical documentation
Qualifications:
Active Registered Nurse (RN) license in NJ
Graduate of an accredited nursing program (BSN preferred)
Current CPR/BLS certification
Valid drivers license and reliable transportation
Previous home care or clinical nursing experience preferred
