Based in ParramattaFull Time Contract to 30th June 2027 with possibility of extensionSCHCADS Award Level 4Do you want to work for a leading national not-for-profit organisation providing dedicated human and social services to a diverse AustraliaWith community at the heart of everything we do our purpose is to create a world in which everyone has access to equal RoleThe Case Manager is responsible for providing holistic case management support to newly arrived refugees and asylum seekers within the Status Resolution Support Service (SRSS) in NSW under contract with the Department of Home Affairs (DHA).The SRSS program is a complex and multifaceted program model that works with asylum seekers in the community that are awaiting the outcome of a visa assessment and are considered highly vulnerable due to their economic visa or health status. This position will be responsible for ensuring the effective and efficient delivery of face to face and remote quality services with refugees and asylum seekers. About YouThe position will report to an SRSS Team Leader and will work within a team of up to 7 Case Managers from a variety of backgrounds and disciplines. Each Case Manager will provide day to day services predominately to families with children and single adult clients over the age of 18. ResponsibilitiesDevelop holistic and client centred case plans with involvement of the client/s. Ensure appropriate exit planning and transition to post SRSS support within an appropriate comprehensive and regular needs and risk assessments of clients against the vulnerability criteria for the SRSS programSupport the clients to develop independent skills and to access appropriate supports in their communityOrganise case conferences for clients with complex needsMaintain accurate digital records of client interactions and services within the SRSS Client Database in line with SSI policies and proceduresEnsure consistently high-level service is provided to asylum seekers through teamwork and support to SRSS colleaguesPrepare client for exit from the service and establish closure with the clientPlease reach out to us for the full Position Description to review the Key Responsibilities and Measures of success. Essential CriteriaDiploma/Degree in Community Services/Welfare or related discipline.
