Medical Social Worker MSW (PRN)

This PRN role gives you the ability to stay in the field support patients and build your own schedule.

What You Will Do

Complete social work assessments in the home
Identify psychosocial financial and environmental needs
Support care planning and discharge coordination
Connect patients and families to community resources
Provide counseling and emotional support
Collaborate with nurses physicians and care teams
Document visits and updates in a timely manner

What You Need

Masters Degree in Social Work (MSW)
Active state social work license
1 year healthcare or home health experience preferred
Strong communication and organization skills
Reliable transportation and willingness to travel (50%)

Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
