Licensed Practical Nurse (LPN/LVN) Long-Term Care (LTC)
Location: Grand Junction CO 81506
Duration: 13-Week Contract
Schedule: 3x12-Hour Variable Night Shifts
Guaranteed Hours: 48 hours/week
Job Overview
A healthcare facility is seeking experienced LPN/LVNs for a Long-Term Care (LTC) / Nursing Home setting. The ideal candidate will be comfortable working night shifts and delivering high-quality patient care in a fast-paced team-oriented environment.
Key Responsibilities
- Provide direct patient care in a long-term care setting
- Administer medications and treatments as prescribed
- Monitor resident conditions and promptly report any changes
- Perform wound care and basic nursing procedures
- Assist residents with activities of daily living (ADLs) and ensure comfort
- Maintain accurate documentation and patient records
- Collaborate with RNs CNAs and interdisciplinary care teams
Required Qualifications
- Active LPN/LVN license (Colorado or compact state)
- Minimum 1 year of experience in LTC or nursing home setting
- Strong clinical and patient care skills
- Ability to work independently during night shifts
