Job Summary

Position: Assistant Program Director/Site Social Worker
Location: Sea Rise Supported Housing - 2760 West 33rd Street Brooklyn 11224
Hours: Full Time - 35 hours per week 10am-6pm Mondays-Fridays
Salary range: $65000 - $70000
Reports to: Program Director
FLSA Status: Exempt


Program Summary:
Founded in 1866 Brooklyn Community Services (BCS) empowers children and families to reach their full potential strengthen families and foster self-sufficiency in adults. With programs in 40 locations BCS serves more than 20000 individuals a year.
Sea Rise One is a 72-unit permanent supported housing program located in Coney Island Brooklyn. The program specifically provides services for Serious and Persistent Mental Ill (SPMI) adults Youth Adults exiting shelter and consumers from the New York State Office for People with Developmental Disabilities (OPWDD).


Position Summary:
The Assistant Program Director/Social Worker will respond to situations where stress depression or any other mental health issues that may create a personal crisis including but not limited to the following: suicidal thoughts emotional trauma serious mental illness anxiety housing crisis. The Assistant Program Director/Social Worker models the best practices of hospitality positive culture and work enthusiastically as part of a team. Additionally this position will work collaboratively with the Program Director to oversee the programs staff outcomes and operational protocols.


Responsibilities:
Conduct routine behavioral risk assessments and safety plans
Provide guidance to clients in dealing with housing-related issues
Recommend appropriate Person-Centered interventions strategies
Provide routine follow-up with clients
Coach residents in establishing and maintaining safe and independent lifestyles
Share information about resident progress needs and problems with other staff
Provide crisis management support as necessary.
Facilitate one-on-one sessions and groups.
Support staff with crisis debriefings and clinical decision-making
Assist in onboarding and training new hires
Facilitate clinical team meetings
Manage a caseload of high-risk clients
Make referrals for clients and linkages
Provide comprehensive clinical services including assessments safety planning and crisis intervention
Develop and implement Wellness Self-Management Services Plans
Serve as an advocate and liaison for person in crisis
Complete documentation in a timely manner in Apricot database
Emergency on-call rotation with Program Director
Follow-up on hospital admissions and discharges
Attend staff meetings and trainings as required by Program Director
Other tasks as may be required


Qualifications Required:
Masters degree required MSW MHC LMSW or LMHC preferred.
Experience working with individuals with mental illness and substance abuse preferably with homeless adults and at-risk populations.
Strong communication and organizational skills; great attention to detail
Ability to develop a relationship with residents while maintaining boundaries
Have a strong sensitivity to cultural differences in order to work well amongst internal and external partners and clients served. Proven ability to work with a range of people from diverse backgrounds and circumstances
Maintain client and staff confidentiality
Comfortable providing services in BCS office and tenants apartments
Computer literacy (Microsoft Word Excel Gmail etc.)
Previous work experience in a social service agency or non-profit organization
Interest in working with underserved populations.
Familiarity electronic data records systems
Bilingual in Spanish preferred
