We are seeking a dedicated Physical Therapist to evaluate plan and implement individualized treatment programs for patients. The ideal candidate will collaborate with interdisciplinary teams to improve patient mobility function and overall quality of life.
Responsibilities:
Evaluate patients and develop individualized treatment plans
Implement therapeutic interventions based on clinical assessment
Establish measurable goals in collaboration with patients and healthcare teams
Monitor patient progress and modify treatment plans as needed
Maintain accurate and timely documentation including progress notes and discharge summaries
Participate in clinical rounds meetings and interdisciplinary discussions
Educate patients and families on treatment plans and recovery strategies
Assist in training and mentoring staff students and new team members
Contribute to program development and quality improvement initiatives
Requirements:
Minimum 2 years of Physical Therapy experience required
Strong clinical assessment and patient care skills
Ability to work collaboratively in a team-based environment
Excellent communication and documentation skills
Education & Licensure:
Bachelors Degree in Physical Therapy (or higher) required
New York State Physical Therapist License (or eligibility) required
