Job Summary

Location:  NG3 NG4 NG5 NG6 NG8 

Pay Rate:13.38 per hour plus 28p mileage 

Shifts available: 07:00 - 22:00 four days per week on-call for additional payment of up to 250.00 per week

Shift Pattern: Week 1: Monday Tuesday Thursday Friday Week 2: Monday Friday Saturday Sunday

Qualifications :

What you need

You would need experience in carrying out initial risk and needs assessments in respect of individual service users either in the service users home or where required in other settings such as hospitals or residential/nursing homes; Work in partnership with service users their family carers advocates healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support.
