About the Role

As a Physical Therapist at Concept Rehab you will provide evidence-based evaluation and treatment to patients recovering from illness injury or surgery in a post-acute rehabilitation setting. Youll collaborate closely with interdisciplinary teams patients and families to help individuals regain mobility strength and independence.

This role is ideal for PTs who value clinical quality teamwork and professional development.

Key Responsibilities

• Evaluate patients within 2448 hours of physician orders
• Develop and implement individualized treatment plans
• Provide skilled patient-centered therapy interventions
• Supervise and collaborate with Physical Therapy Assistants
• Communicate effectively with physicians nursing families and care teams
• Participate in interdisciplinary and care planning meetings
• Document evaluations treatments and progress accurately and timely
• Support discharge planning and caregiver education
• Participate in staff education and in-service training as appropriate

Qualifications

• Bachelors Masters or Doctorate in Physical Therapy
• Active Ohio Physical Therapist license
