Job Summary

Synergy Care is hiring a full-time Physical Therapist for an outpatient clinic serving an AL/IL community in Fort Worth TX!

We are looking for a team-oriented clinician who is passionate about providing quality patient care and making a difference in the lives of others.

Job Responsibilities:
Evaluate patients and develop individualized treatment plans
Provide ongoing treatment and discharge planning
Supervise assistants and aides
Participate in case management
Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines
Represent Synergy Cares commitment to excellence in every patient and facility interaction

Qualifications:
Graduate of an accredited Physical Therapy program (or graduating within 6 months)
