About the roleThis newly identified Aboriginal Health Practitioner role has been established to strengthen access to culturally safe primary health care for our community.
Working within the Clinical Services team and reporting to the Clinic Practice Manager this role delivers comprehensive culturally appropriate primary health care services in both clinic and community settings. The role prioritises improved access to care and supports activities across health promotion prevention chronic disease management eye ear and oral health and palliative care.
Key responsibilitiesHealth Service Delivery & Telehealth
- Provide clinical services with the Scope of Practice for Aboriginal Health Practitioners and appropriate qualifications including:
- Contributing to Aboriginal Health Checks (e.g. basic physical examination consulting with more experienced clinical staff as necessary and provide advice on the treatment and management of clients health conditions).
- Conduct 1:1 patient information and education activities including providing healthy lifestyle advice and information about services available through other agencies where appropriate and assist the client in organising referrals (including appointments and transport) to other agencies as necessary.
- Advocating on the clients behalf in relation to their current health concerns if requested i.e.; Telehealth Allied Health Specialists etc.
- Submit the required claim forms for services provided following claims under their own provider to a person who is of Aboriginal and Torres Strait Islander descent.
- Booking and organising Telehealth appointments.
- Must have or willing to complete Genexpert and QAAMS training provided by MDAS within 12 months of starting date.
- Must be willing to complete Immunisation course provided by MDAS.
Planning and Reporting Compliance
- Complete all required data entry and reporting responsibilities outlined in the Key Performance Indicator (Quality Improvement) Summary developed in consultation with Clinical Coordinator and in accordance with external reporting requirements (Department of Health VACCHO others as required).
- Recalls and reminders to be completed weekly.
- Ensure documentation and record keeping occurs in line with professional standards and organisational requirements.
Program Management
- Assist other staff members in the development presentation and monitoring of health promotion and education programs.
- Foster and maintain relationships with other agencies in order to benefit clients and the organisation as a whole.
- Promote and facilitate clients self-management of health and wellbeing through assessment and education maximising Medicare item numbers for Aboriginal Health workers to facilitate increased health outcomes.

- Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice or Community)
- Current registration with AHPRA as an Aboriginal Health Practitioner including professional indemnity cover
- Current Australian Drivers Licence
- Employee Working with Children Check
- National Police Check
