Pay range: $35.20 - $41.15 per hour depending on years of experience
Centurion is proud to be the provider ofcomprehensive services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Full-time Day Shift Discharge Planner to join our Substance Use Disorder (SUD) team at Maryland Correctional Institutionlocated in Hagerstown Maryland.
The SUD Program Discharge Planner is responsible for coordinating a patients release from the substance use program upon release or leave to another facility. This person is responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders as well as identifying the need to return patients to referral source at the time of discharge.
Shift: Full time Day shift (40 hours per week)
Qualifications
- Bachelors Degree in Psychology Social Work Counseling or related field from an accredited educational institution; Masters Degree preferred
- Minimum of two (2) years of experience working in release planning preferred
- Clear written and verbal communication skills
- Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR)
- Corrections forensic or criminal justice experience preferred
- Ability to obtain a security clearance to include drug screen and criminal background check
