Job Summary

Santa Rosa Post Acute is a leading Skilled Nursing Facility (SNF) dedicated to delivering high-quality compassionate care to our residents. We provide comprehensive rehabilitation services to support our patients recovery and improve their overall well-being. Our team is committed to fostering a culture of excellence and professional development and we are currently seeking an experienced Physical Therapist (PT) to join our team.

Position Overview:
As a Physical Therapist at Santa Rosa Post Acute you will be responsible for assessing and treating patients to help them regain physical function and improve mobility. Working within a collaborative healthcare team you ll design and implement individualized treatment plans that promote independence and enhance the quality of life for our residents.

Key Responsibilities:

  • Perform thorough evaluations to assess patients physical abilities and develop personalized treatment plans.
  • Provide physical therapy interventions to improve strength mobility and functional abilities.
  • Educate patients and their families on therapeutic exercises safe movement techniques and home modifications.
  • Collaborate with the interdisciplinary team including occupational therapists nurses and physicians to coordinate care and optimize patient outcomes.
  • Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
  • Maintain accurate and timely documentation of patient assessments progress notes and treatment plans in accordance with facility policies.
  • Ensure that care plans align with regulatory requirements and best practices in physical therapy.

Qualifications:

  • Bachelor s Master s or Doctoral degree in Physical Therapy from an accredited program.
  • Valid Physical Therapy license in State.
  • Experience working in a skilled nursing facility or similar healthcare setting preferred.
  • Knowledge of physical therapy techniques therapeutic modalities and adaptive equipment.
  • Excellent communication interpersonal and problem-solving skills.
  • Ability to work both independently and collaboratively in a team environment.
  • Compassionate patient-focused approach to care.
