Position Overview:
As a Physical Therapist at Santa Rosa Post Acute you will be responsible for assessing and treating patients to help them regain physical function and improve mobility. Working within a collaborative healthcare team you ll design and implement individualized treatment plans that promote independence and enhance the quality of life for our residents.
Key Responsibilities:
- Perform thorough evaluations to assess patients physical abilities and develop personalized treatment plans.
- Provide physical therapy interventions to improve strength mobility and functional abilities.
- Educate patients and their families on therapeutic exercises safe movement techniques and home modifications.
- Collaborate with the interdisciplinary team including occupational therapists nurses and physicians to coordinate care and optimize patient outcomes.
- Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
- Maintain accurate and timely documentation of patient assessments progress notes and treatment plans in accordance with facility policies.
- Ensure that care plans align with regulatory requirements and best practices in physical therapy.
Qualifications:
- Bachelor s Master s or Doctoral degree in Physical Therapy from an accredited program.
- Valid Physical Therapy license in State.
- Experience working in a skilled nursing facility or similar healthcare setting preferred.
- Knowledge of physical therapy techniques therapeutic modalities and adaptive equipment.
- Excellent communication interpersonal and problem-solving skills.
- Ability to work both independently and collaboratively in a team environment.
