What Youll Be Doing
As a Home Health Physical Therapist you will help patients regain strength mobility balance and independence in the comfort of their own homes. You will assess patient needs design individualized care plans deliver therapy services and collaborate closely with other healthcare professionals to ensure strong outcomes.
Key Responsibilities
Perform in-home physical therapy evaluations and develop customized treatment plans
Provide therapeutic exercises mobility training and hands-on interventions
Educate patients and caregivers on home exercise programs safety and long-term wellness strategies
Collaborate with nurses occupational therapists physicians social workers and other care team members
Document progress treatment updates and outcomes in the EMR accurately and on time
Travel to patient homes within your assigned territory
Maintain strong clinical quality professionalism and compliance standards
What Were Looking For
Active Oregon Physical Therapist license
Masters or Doctorate in Physical Therapy preferred
New graduates welcome to apply
Home health experience is a plus but not required
Current CPR/BLS certification
Valid drivers license reliable transportation and proof of auto insurance
Strong communication patient education and clinical decision-making skills
Ability to work independently and manage a dynamic caseload

A Typical Day in This Role
Your day will involve reviewing your caseload traveling to patient homes conducting evaluations or treatment sessions educating patients and families and documenting care in the EMR. Youll also stay connected with the broader care team to adjust treatment plans and ensure each patient receives coordinated high-quality support.
