Title: LTSS Service Care Manager
Location: Hybrid (2days remote & 3 days in home member visits required) Cameron County
Duration: 6 months (Potential extension)
Shift: 1st (8am-5pm) Mon to Fri
Payrate: $30 an hour to $33 an hour
Description:

- To complete WLI/Tasks
- Conduct Biannual visits and document findings
- Coordination with multi-disciplinary teams
- Follow up calls to members
- Any other projects that may arise
Typical Day in the Role
- Meet with Team Lead and Manager. Trainings will need to be completed.
- Shadowing of other Service Coordinators
- To complete WLI/Tasks
- Conduct Biannual visits and document findings
- Coordination with multi-disciplinary teams
- Follow up calls to members
- Any other projects that may arise
- Expectations are to stay within TAT and meet documentation guidelines.
Candidate Requirements:
Education/Certification Required: Licensed Practical Nurse or Licensed Vocational Nurse
Preferred: with case management and/or home health experience
Licensure Required: Licensed Practical Nurse or Licensed Vocational Nurse
Years of experience required: 1-2 years of case management or home health experience
Disqualifiers: Must be bilingual
Additional qualities to look for: time management customer service attention to detail
