Physical Therapist For Home Health

Physicians Choice Home Health is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team serving patients in the South Bay of Los this role you will provide high-quality physical therapy services to patients in the comfort of their own homes helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.

What Youll Do

• Provide in-home physical therapy evaluations and treatments
• Assess patients strength mobility balance and ability to function safely at home
• Create and update individualized plans of care that support independence and recovery
• Help patients regain mobility and confidence in their home environment
• Educate patients and caregivers on exercises safety and fall prevention
• Document visits and evaluations in WellSky/Kinnser in a timely manner
• Collaborate with nurses therapists and the broader care team to support patient outcomes
• Supervise Physical Therapy Assistants (PTAs) and support their clinical development
• Participate in care planning case conferences and discharge planning

• Graduate of an accredited Physical Therapy program (Bachelors Masters or DPT)
• Active California Physical Therapist license
• CPR/BLS certification
• Strong communication and organizational skills
• Reliable transportation and willingness to travel to patients homes
• Comfort working independently in a home health setting
• Home health experience preferred
• Experience charting in Kinnser/Wellsky preferred but not required

• $40-110/hour depending on visit/task type
• Mileage reimbursement

Salary Range Disclaimer: The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future and employees will be notified of these changes.
