Job Summary
Clinical Social Worker - Home Health
Care IV Home Health is now hiring a PRN Clinical Social Worker for our Little Rock territory. Seeking a Social Worker to visit our home health patients to assess their medical social services needs & guide them through the process in conjunction with their physicians orders.

Duties include but are not necessarily limited to:
- Provide rehab and supportive casework geared towards restoring patients to their optimal level of social emotional & health wellness
- Help patients utilize appropriate family & community resources
- Refer patients to appropriate health & welfare agencies
- Consult & coordinate services with physicians and other members of the patients care team
- Prepare clinical progress notes & incorporate into clinical record
- Participate in discharge planning & in-service programs
Required education & experience includes:
- Masters degree in social work from a college accredited by the Council of Social Work Education
- Arkansas social worker license that is valid and in good standing with the licensing board
- 6 months of professional social work experience in a skilled medical environment such as home health hospital rehab facility clinic or related
- As a healthcare provider in compliance with CMS guidelines & conditions of participation we require a COVID vaccine as a condition of employment (absent reasonable accommodations)
