Job Description:
We are seeking a dedicated Physical Therapist for a home health setting. This role involves providing one-on-one care to patients in their homes focusing on improving mobility strength and overall functional independence.
Key Responsibilities:
- Evaluate and treat patients in a home health environment
- Develop and implement individualized care plans
- Manage a daily caseload of up to 6 patients
- Complete accurate and timely documentation including OASIS assessments
- Coordinate care with interdisciplinary healthcare teams
- Educate patients and caregivers on treatment plans and progress
Requirements:
- Active Oregon Physical Therapist license
- Minimum 1 year of skilled home health experience (required)
- OASIS experience required
- BCLS certification required
- Ability to work independently and manage time effectively
- Reliable transportation required for patient visits
