What Youll Do

• Deliver high-quality in-home physical therapy services aligned with Elara Carings mission and standards

• Evaluate patient functional needs establish measurable goals and adjust care plans to achieve optimal outcomes

• Design and implement individualized plans of care with patient and caregiver involvement

• Collaborate with interdisciplinary team members to support patient progress

• Provide direction to support staff to help patients meet functional goals

• Maintain accurate timely documentation including evaluations treatment plans and progress notes

• Serve as a clinical resource and advocate for physical therapy within the community

What Youll Need to Be Successful

• Graduate of an APTA-approved Physical Therapy program

• Current unrestricted Physical Therapist license in the state of practice

• Minimum one (1) year of experience in a healthcare setting

• Ability and willingness to travel within the assigned coverage area (up to 50%)

• Reliable transportation valid drivers license and current auto insurance

• Ability to sit stand bend lift and move intermittently; must be able to lift 50100 lbs
