What You Will Do

• Manage a home health caseload with full autonomy
• Complete Start of Care OASIS and routine visits
• Use HomeCare HomeBase (HCHB) for documentation
• Build care plans based on patient needs
• Educate patients and families in the home
• Collaborate with physicians and care teams
• Deliver high-quality patient-centered care

What You Need to Succeed

• Active RN license
• Home Health experience required
• Strong HCHB (HomeCare HomeBase) experience required
• OASIS documentation experience
• Ability to manage your own schedule and territory
• Strong clinical judgment and communication skills

What Sets You Apart

• You know HCHB and can hit the ground running
• You are confident in Start of Care visits
• You value independence but want strong support behind you

Additional Requirements

• Valid drivers license
• Ability to travel within territory
• Ability to frequently lift push pull and support up to 50 pound
