Medicare : All providers must be eligible to see Medicare patients (i.e. not opted out) and must be enrolled in Medicare. If not already enrolled Medicare applications will be submitted concurrently with credentialing to help ensure approval by the start date. Rate Structure: Rates are flat and all-inclusive . Callback call overtime travel and holiday pay are included in the flat rate there is only one rate that can be billed. All expenses must be built into the hourly or shift rate depending on the assignment. No separate billing for travel overtime holidays or other add-ons. Credentialing Timeframe (with clean background/MP/criminal history): AZ Licensed : 6 weeks IMLC : 2 months Not Licensed in AZ : 3 months Credentialing can begin while license is in process. Submission Restriction : Agency ownership is 1 year from date of submission; providers cannot be presented by another agency for one year after submission.
