We are seeking an Advanced Practice Provider to join one of the nations most integrated healthcare systems with over 1300 providers in Indianapolis Indiana.

Highlights

Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Indianapolis IN. The most comprehensive palliative care program of its kind in Indiana our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.

• 1.0 FTE position weekdays
• This provider will work autonomously and collaboratively with our palliative care multi-disciplinary teams as a float provider at our network hospitals (Community Hospitals North East South Community Heart & Vascular Hospital Community Howard Regional Hospital and Community Hospital Anderson) to deliver palliative and health care services to patients.
• Focus on the comfort measures of patients end of life care symptom management as well as assisting with consults and new admissions.
• Flexibility to travel between hospitals in Indianapolis

Qualifications

Ideal candidates will:

• Possess 1-3 years Experience as a licensed Nurse Practitioner in Palliative Care
• Be comfortable assessing patients independently
• Be compassionate caregivers with excellent organizational communication and critical thinking skills
• Possess highly developed interpersonal and group dynamic skills as well as proven skills as a consultant/educator to friends families and loved ones

Licensure & Certifications

Educational background consistent with requirements for licensure and certification within the state of Indiana
