- Full-Time positions - based in Rocky Bay Hubs from Joondalup to Mandurah

About the Role
- Provide person-centred evidence-based behaviour support within a holistic individualised service model
- Work closely with individuals families and caregivers to build shared understanding and meaningful outcomes
- Develop and implement strategies that enhance quality of life and reduce behaviours of concern
- Manage a diverse caseload including complex presentations with the support of a collaborative team
- Contribute to a multidisciplinary approach working alongside allied health professionals and external stakeholders
What Youll Be Doing
- Conduct functional behaviour assessments and develop behaviour support plans
- Design and implement evidence-based interventions including restrictive practices in line with NDIS guidelines
- Apply a range of tools and strategies to meet individual needs
- Monitor and evaluate the effectiveness of behaviour support plans
- Build relationships with families caregivers and external stakeholders
- Maintain accurate and timely documentation

Who Were Looking For
- A compassionate and skilled practitioner with a passion for person-centred support
- Experience working with complex behaviours and diverse needs
- Ability to work collaboratively within a multidisciplinary team
- Commitment to ongoing learning and development
- Current NDIS Positive Behaviour Support (PBS) registration (or eligibility to obtain)
