Classification Level: 5 ($51.27 to $53.59 per hour)
THE ROLE
At Kyabra the Case Management and Coordination (CMC) team provides casework support to a range of community members towards achieving their personal aspirations/goals and development using a strength-based approach. Service work is based on an integrated model whereby the team works alongside community members to support them in navigating the systems and circumstances that significantly impact their lives such as child protection housing living with a disability (NDIS) living on a low income healthcare (mental and physical) education and legal services.
In conjunction with the Manager the Coordinator for NDIS will support the day-to-day operations of the team and assist in facilitating the delivery of high-quality services. This will include:
- Being responsible for a specialist service area with an expectation that the successful candidate will provide expert guidance and support as it relates to achieving effective outcomes
- Providing support and supervision to specified Case Workers
- Data collection and reporting (in conjunction with other organisational services)
- Building and maintaining positive relationships with stakeholders and
- Remaining informed about the industry while maintaining compliance accordingly.
This is a fixed-term contract to cover a period of maternity leave.
Hours of Work
Applicants with the capacity to work full-time hours (76h 00m per fortnight) are preferred however applicants with part-time availability will be considered. Part-time applicants must be available to work a minimum of 60h 48m per fortnight.
KEY SELECTION CRITERIA
Qualifications & Training
- Tertiary qualification (bachelors degree or higher) in human services and/or another relevant field.
Skills & Experience
- Demonstrated knowledge of and experience working in case management in the community service sector including the use of client management systems as well as experience in any of the following areas: needs assessments case management advocacy and coordinating support services for people experiencing multiple complexities.
- Demonstrated leadership skills and experience in supervising and supporting staff.
- Demonstrated understanding of issues/barriers facing community members including ways to resolve or improve these issues and empower community members particularly as it relates to people who live with a disability.
- Demonstrated understanding of the Disability Services Act and other relevant human services legislation particularly as it relates to supporting people who live with a disability.
- Understanding of and commitment to the principles of social justice as well as a demonstrated capacity to work in ways that focus on peoples strengths.
- Strong communication interpersonal and organisational skills including an ability to work autonomously or as part of a team and adapt to various circumstances as necessary to support the organisation and/or community members.
History Checks & Clearances
The successful applicant will be required to apply for certain mandatory Personal and Criminal History Checks. This position is required to hold a satisfactory/positive result for the following:
- Blue Card (Working with Children Check)
- NDIS Worker Screening
- LCS2 (Personal and Criminal History Check) and
- A valid open drivers license (Queensland).
