What Youll Do In This Role:
LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) this role you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.

What Youll Do In This Role:
LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) this role you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.
Key Responsibilities
- Provide Hands-On Care in Their Home Assist with personal care (ADLS) like bathing grooming dressing and toileting while following the participants Life Plan.
- Maintain Comfort & Care Support housekeeping tasks such as laundry sweeping and grocery shopping
- Support Mealtime Help participants enjoy their meals and stay hydrated for good health.
- Keep Things Moving Support mobility including turning positioning and safe transfers.
- Perform Vital Checks Measure and record vital signs weight and intake/output.
- Maintain Comfort & Care Keep rooms tidy cozy and well-stocked with essentials.
- Provide Responsive Support Respond with kindness and compassion.
- Monitor with Watchful Eyes Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
- Demonstrate Respect & Professionalism Maintain confidentiality while delivering top-notch care.

Skills Knowledge & Expertise
- Certified Home Health Aide (HHA) or Certified Nurse Aide (CNA) per state requirements required.
- Previous experience providing care to older adults preferred.
- Reliable transportation and valid drivers license required.
- High school diploma or GED required.
- Excellent communication and observation skills.
