Home Health Licensed Practical Nurse (LPN)

Your Role as an LPN on the Elara Team
Partner closely with your RN Case Manager to deliver care aligned with each patients individualized Plan of Care
Provide skilled nursing visits reinforcing care plans and patient goals in the home setting
Communicate changes in condition concerns and updates to your RN partner to ensure coordinated proactive care
Contribute your clinical insightyour observations help shape and improve the Plan of Care
Educate patients and caregivers on medications treatments and self-care techniques
Help prevent avoidable hospitalizations through early intervention and patient support
Maintain accurate timely documentation of visits and care provided
Build trust with patients and families through compassionate respectful care
What You Bring
Graduate of an accredited Certificate Diploma or Associate School of Nursing
Current State License as a Licensed Practical Nurse (LPN/LVN)
At least 1 year of clinical experience
Home health or hospice experience preferred but not required
Strong communication skills and a team-first mindset
Ability to work independently while staying closely connected to your RN partner
Valid drivers license insurance and reliable transportation
Willingness to travel within the assigned territory
Physical Requirements:Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
