Your Role as an LPN on the Elara Team

• Partner closely with your RN Case Manager to deliver care aligned with each patients individualized Plan of Care

• Provide skilled nursing visits reinforcing care plans and patient goals in the home setting

• Communicate changes in condition concerns and updates to your RN partner to ensure coordinated proactive care

• Contribute your clinical insightyour observations help shape and improve the Plan of Care

• Educate patients and caregivers on medications treatments and self-care techniques

• Help prevent avoidable hospitalizations through early intervention and patient support

• Maintain accurate timely documentation of visits and care provided

• Build trust with patients and families through compassionate respectful care

What You Bring

• Graduate of an accredited Certificate Diploma or Associate School of Nursing

• Current State License as a Licensed Practical Nurse (LPN/LVN)

• At least 1 year of clinical experience

• Home health or hospice experience preferred but not required

• Strong communication skills and a team-first mindset

• Ability to work independently while staying closely connected to your RN partner

• Valid drivers license insurance and reliable transportation

• Willingness to travel within the assigned territory

• Physical Requirements:Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
