Flexible PRN opportunity for Registered Nurses who enjoy admissions assessments and helping patients transition safely home after hospitalization.

What Youll Do
As a PRN Admissions Registered Nurse you will complete home health admissions and initiate the patients plan of care.
You will:
Complete Start of Care and Admission visits for home health patients
Perform comprehensive clinical assessments and documentation
Initiate the physician ordered plan of care
Educate patients and families on medications treatment plans and expectations
Communicate patient condition and findings with physicians and care teams
Identify changes in patient condition and adjust care plans when needed
Ensure documentation meets regulatory and quality standards

What Youll Bring
Current Registered Nurse (RN) license in the state
Graduate of an accredited nursing program
1 year clinical nursing experience
Strong patient assessment and documentation skills
Ability to travel within the branch coverage area
Reliable vehicle valid drivers license and current auto insurance
Preferred:
Home Health RN or Hospice RN experience
Experience with Homecare Homebase (HCHB)
