Plans and manages patient care according to each patients needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs including pulse blood pressure temperature and respiration. Provides routine care for patients.
Responsibilities
Provide 1:1 care for client always following clients plan of care while maintaining clients dignity and independence
Monitoring clients vital statistics and reporting abnormal findings to the case manager and physician assessments
Performs medication administration feedings oxygen ostomy care as needed per clients plan of care and provided assessments of results
Complete routines as established by other care professionals such as PhysicalTherapist Occupational Therapist and Speech Therapist
Ensures infection control policies are maintained at all times reports possible communicable diseases or infections and ensures the overall implementation and practice of infection control procedures are implemented
Accurately documents nursing actions of all care given and communicates with family case manager and physicians as well as documents the progress and outcomes for established goals and informs physician case manager and families of changes in clients medical condition and needs
Orders supplies and medications as needed to ensure adequate amounts are on hand at all times
Ensures all patient needs are being met in accordance with the instructions of the physician.
Monitors incidents involving individuals. Ensures incident reports are completed timely accurately and case managers are informed; examines the individual following the incident
Ensures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repair
Other duties as assigned
Qualifications
Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse
Knowledge of individual service plans including understanding of client rights and health needs
Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice
CPR Certified
Excellent interpersonal skills including customer service patience empathy and compassion
In-depth understanding of patient care policies and regulations
Familiarity with symptoms and signs of improvement for several medical conditions
Moderate amount of travel between facilities may be required
