• Assess & Plan
  
  
  • Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
    
    
• Coach & Supervise
  
  
  • Orient caregivers to each Care Plan perform field competency checks and ensure ongoing compliance.
    
    
• Monitor & Document
  
  
  • Track vitals condition changes and interventions in our HIPAA-secure EMR; escalate concerns promptly.
    
    
• Collaborate & Advocate
  
  
  • Coordinate with physicians therapists and schedulers to keep care seamless and client-centered.
    
    
• Quality & Compliance
  
  
  • Maintain audit-ready client files collect initial service payments and uphold state home-care regulations.
    
    

• Active Registered Nurse (RN) license in NJ
  
  
• Bachelors in Nursing or related human-services field OR 2 years geriatric/home-care experience
  
  
• Solid clinical assessment skills and a passion for problem-solving
  
  
• CPR/First Aid certification and a valid drivers license
  
  
• Warm professional communication that builds trust with families and field staff
