Your Role: Home Health LVN

As a Home Health Licensed Vocational Nurse (LVN) you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). This role is ideal for LVNs experienced or interested in home health geriatric care dementia care and community-based nursing.

Primary Responsibilities

Provide skilled home health nursing care based on individualized care plans
Deliver care to geriatric and senior patients in home-based and assisted living settings
Accurately document visits treatments and assessments in the EMR system
Collaborate with physicians RNs families and interdisciplinary care teams
Support patients with chronic conditions recovery and dementia care needs
Ensure compliance with California home health regulations and company policies
Promote patient independence safety and overall well-being

Qualifications

Active California Licensed Vocational Nurse (LVN) license in good standing
Graduate of an accredited vocational nursing program
Minimum 1 year of nursing experience (Home Health experience preferred)
Experience with geriatric patients senior care or dementia care preferred
Valid drivers license reliable transportation and auto insurance
Strong communication time management and documentation skills
Comfortable using electronic medical records (EMR systems)
