Job Summary

About the role

We are seeking an Occupational Therapist to join our small rural health service playing a vital role in supporting older people across the Tallangatta community and surrounding region. This position has a strong focus on community and aged care services supporting older adults to remain safe independent and engaged in their own homes for as long as possible.

Working as part of a close-knit multidisciplinary team you will manage a predominantly aged care caseload across home and community settings. The role includes comprehensive functional assessments care planning and delivering practical interventions such as equipment prescription ADL support home modifications falls prevention and cognitive screening.

A key component of the role is supporting clients to navigate aged care services including Support at Home funding pathways while working closely with families carers and local providers to coordinate care and optimise outcomes.

As part of a small rural service you will build meaningful relationships within the community and contribute to a flexible person-centred model of care that ensures continuity across the consumer journey. While the primary focus is aged care there may be opportunities to contribute to broader community programs including NDIS over time.

This is an excellent opportunity for an Occupational Therapist who is passionate about aged care values strong community connections and is motivated to make a genuine difference in a rural setting.

This a permanent part time position of 2 days per week.

Tallangatta Health Service is a 61 bed small rural public health service providing a mix of acute sub-acute aged care residential and community services to Tallangatta and the surrounding district. We pride ourselves on being a customer focused progressive provider of care to our community patients and residents with a multidisciplinary focus.

About you

  • Passion for supporting consumers to live life to their potential
  • Bachelor of Occupational Therapy with current AHPRA registration
  • Excellent interpersonal and written and verbal communication skills
  • Demonstrated ability to provide written and verbal reports and commitment to continuous quality improvement and development.
  • Demonstrated industry experience and practical working knowledge of the Allied Health Service preferably in a community or aged care setting is desirable but not essential.
