About the Role
An exciting opportunity currently exists for a Grade 2 Speech Pathologist to join the Community Rehabilitation and Home Based Allied Health team on a part time fixed-term basis. The Speech Pathologist will deliver high quality person-centred goal-orientated home and centre-based care within a supportive multidisciplinary team.
Classifications for this position will range from VW1 - VW4 ($90235.60 - $103724.40) per annum pro rata.
Your Contribution
- Bachelor of Speech Pathology or equivalent
- Eligibility for Speech Pathology Australia membership
- Sound knowledge and application of speech pathology practice and principles in the area of rehabilitation and aged care
- Demonstrated highly developed clinical management skills
- Experience in management of adults with communication and swallowing disorders of neurological origin
- Understand and support the principles of Health Independence Program continuum of care for clients particularly in relation to individualized time-limited goal-centred episodes of care that aim to improve health outcomes
- Highly developed organization planning and problem solving skills
- Demonstrated ability to work independently
- Highly developed leadership and interpersonal skills that incorporate excellent communication
- Evidence of ongoing education/professional development relevant to Speech Pathology practice.

- Current Victorian drivers licence

About the Departments - Community Rehabilitation and Home Based Allied Health
Community Rehabilitation
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow through delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centred care planning.
St Vincents Hospital Melbourne (SVHM) HIP consists of integrated hub sites across several locations including in Fitzroy Kew North Richmond and Fairfield.
HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH). This position is part of the HIP Community Rehabilitation team. Community rehabilitation is currently provided from our centres at Kew and Fairfield.
The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness are frail or chronically ill to regain optimal function and to allow people to maximise their independence. The programs offer a supportive and dynamic team environment which focusses on patient-centred care.
Home Based Allied Health
St Vincents Virtual & Home have admitted services across Acute and Subacute streamed Units including Hospital in the Home Transition Care Bed Based. Non admitted services including District Nursing Residential in Reach Palliative Transition Care Program Home Based Allied Health.
The St Vincents Home Based Allied Health team is funded by the Commonwealth Home Support Program (CHSP) and the Victorian Home and Community Care Program for Younger People (HACC-PYP) and focuses on restoring or maintaining peoples independence improving functioning and wellbeing to support them to remain living at home. The team includes: Physiotherapy Occupational Therapy Speech Pathology Podiatry Dietetics and an Allied Health Assistant. The Home Based Allied Health Service is based at St Georges Health Service (SGHS) and the position requires staff to undertake home visiting duties.
The Transition Care Program is jointly funded by the Commonwealth and State and Territories. St Vincents Transition Care Program is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care. The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
