Job Summary
Under the direction of the Trauma Nurse Director the Patient Service Navigator coordinates multidisciplinary follow-up care for trauma and emergency general surgery patients. This role focuses on patient- and family-centered care supporting self-determination and addressing medical psychosocial and resource needs through comprehensive assessment and care planning. Working collaboratively with a multidisciplinary healthcare team the Navigator serves as a liaison among specialty services to ensure efficient scheduling communication and continuity of care across services such as Trauma Emergency General Surgery Orthopedics Neurosurgery and others.Qualifications
Principal Duties and Responsibilities
trauma and emergency general surgery patients with scheduling multidisciplinary follow up clinic appointments preferably before they are discharged from the hospital.
2. Coordinates with multidisciplinary services to help patients navigate the system decrease barriers to follow up and schedule these appointments.
3. Works closely with hospital care coordination and inpatient social work to identify and resolve potential social or financial challenges for patients needing follow up care.
4. Assists with connecting the patient to community resources including primary care providers behavioral health and community financial resources. Assists with enrolling patients in qualifying governmental or insurance programs (FMLA MassHealth) if applicable and not already in place.
5. Follows up with patients primary care physicians or referring providers about their recent hospital encounter highlighting any new diagnoses and incidental findings.
6. Routinely visits with trauma and emergency general surgery patients on the inpatient unit to ensure they are receiving the attention they need and expect. 6. Alerts program leadership to patient service issues and any areas in need of improvement.
7. Contributes to the continuous performance improvement of the trauma and emergency general surgery programs.
8. Provides relevant updates at the quarterly Trauma Performance Improvement and Patient Safety (PIPS) committee meetings.
9. Assists with any other trauma-related project or initiative as needed at the discretion of the Trauma and Acute Care Surgery Nurse Director.

Masters Degree Social Work required

Licensed Clinical Social Worker LCSW - State License required
Experience
Clinical experience in medical setting 0-1 year required

Demonstrates fundamental skills in formulation assessment of risks crisis intervention completion of psychosocial treatment plans and appropriate documentation.
May need assistance and guidance in ensuring the patients needs safety measures and concerns are brought forward.
Emerging knowledge of internal and external resources.
Developing and demonstrating interpersonal collaboration in a medical setting- inpatient or outpatient.
May need coaching/guidance in this area.
Requires ability to work effectively with physician nursing secretarial and ancillary staff.
Ability to study issues systematically and generate innovative solutions to complex problems.
Ability to focus detailed concentrated effort to multiple tasks and re-establish priorities as necessary.
Ability to effectively respond to time sensitive issues.
Demonstrated ability to manage a busy office environment within the context of a large medical center and healthcare delivery system.
