Job Summary
Position Overview
Inabia is seeking a compassionate and motivated Physical Therapist to provide rehabilitation therapy services to residents within a senior living and skilled nursing community. The therapist will help residents maintain or regain mobility independence and overall physical health.
Key Responsibilities
- Conduct comprehensive physical therapy evaluations to determine patient mobility limitations and rehabilitation needs.
- Develop individualized treatment plans to improve strength flexibility balance and coordination.
- Provide therapeutic interventions including mobility training strength exercises manual therapy and balance rehabilitation.
- Assist residents recovering from surgery illness stroke or chronic conditions.
- Monitor therapy progress and update treatment programs based on patient performance and goals.
- Educate residents and caregivers on safe mobility practices fall prevention and home exercise programs.
- Maintain accurate therapy documentation including evaluations treatment notes and discharge summaries.
- Work collaboratively with interdisciplinary care teams including nursing staff physicians occupational therapists and speech therapists.
- Participate in care planning meetings and support safe discharge planning.
Requirements
- Active Physical Therapist License
- Degree from accredited Physical Therapy program
- Experience in rehabilitation or senior living environment preferred
