Director of Intake Home Health

Job Responsibilities
- Monday Friday (Standard Business Hours)
- Oversee Daily Intake Operations Including ReferralsAdmissions and Patient Onboarding
- Supervise and Lead Intake Staff to Ensure Timely Start of Care
- Manage Intake Logs Reports and Patient Flow Tracking
- Communicate with Referral Sources to Clarify Patient
- Needs and Coordinate Care
- Collaborate with Branch Directors and Business Development Teams
- Ensure Accurate and Timely Data Entry into EMR Systems
Monitor Compliance with PDGM Guidelines and Documentation Standards - Assist with Strategic Planning Budgeting and Process Improvement Initiatives
- Participate in Quality Assurance and Performance Improvement Activities
- Act as a Liaison Between Intake Clinical Teams and Executive Leadership
Job Requirements
- Experience in Home Health Required
- Leadership or Supervisory Experience Required
Strong Knowledge of Intake Processes Referrals and Admissions - Experience with EMR Systems and Healthcare Documentation Standards
- Familiarity with PDGM Guidelines Preferred
- Strong Organizational and Communication Skills
- Ability to Thrive in a Fast-Paced Environment
