Your role and responsibilities
As a Transforming Care Navigator youll act as a catalyst for change working directly alongside autistic children young people (up to age 25) and those with a learning disability as well as their families. Youll help ensure their voices are heard their needs are understood and the right support is in place to help them thrive in their community and avoid unnecessary hospital admissions.
A typical day might include visiting a young person at home or in hospital working with families and professionals to shape clear and practical plans and challenging systems where support isnt working as it should.
On aday-to-daybasis you might:
- build trusted relationships with children young people and their families
- act as a key point of contact coordinating multiagency support where required
- support families to navigate health care and education systems
- ensure actions from Care Education and Treatment Reviews (CETRs) are followed through
- escalate concerns and gaps in provision to senior colleagues and commissioners
- hold accurate records and contribute to quality assurance activity
- work within safeguarding statutory and professional frameworks at all times.

You will need
Were looking for someone who is passionate about improving outcomes for children young people and families and who brings warmth resilience and curiosity to their practice.

- a suitable graduatelevel qualification or equivalent relevant experience
- experience of working directly with children young people and families
- a strong understanding of safeguarding and risk and when to escalate concerns
- confident communication skills and the ability to build trust with families and professionals
- a solutionfocused collaborative approach to working
- a commitment to equality diversity and inclusion.
Experience of working with autistic children and young people those with a learning disability or within a Transforming Care context is desirable but if you feel you could succeed in this role wed still love to hear from you.

The Team
The Keyworker Function Team supports children and young people who are autistic and/or have a learning disability and who may be at risk of a mental health hospital admission. Were proud of our focus on listening advocacy and partnership working and were passionate about helping young people stay connected to their communities.
We work collaboratively support one another and value reflective practice. Officebased working usually takes place alongside flexible and hybrid arrangements.
