We are seeking a dedicated and compassionate Community Habilitation Worker to join our team. As a Community Habilitation Worker you will provide essential support and assistance to individuals with special needs. Your role will involve working directly with clients and their families to ensure their well-being and promote their independence.

Responsibilities: - Provide direct support and care to individuals with special needs including assisting with daily activities and personal care - Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors - Collect data on client progress and behavior using specified methods and tools - Collaborate with clients Circle of Support to develop and implement individualized care plans - Assist clients in developing essential life skills such as communication socialization and self-care - Provide transportation for clients to appointments activities or community outings - Create a safe and supportive environment for clients ensuring their physical and emotional well-being - Maintain accurate documentation of client interactions progress and any incidents that occur

Skills: - Experience or knowledge in special education or caregiving is highly preferred - Proficiency in data collection methods and techniques - Valid drivers license and reliable transportation for client transportation - Knowledge of behavioral health principles and strategies - Experience in direct support or childcare roles is beneficial - Ability to work effectively with individuals with special needs

Job Type: Part-time

Pay: $24.00 per hour

Expected hours: up to 6 hours per week
