University Home Care Inc is seeking a dedicated and compassionate Hospice RN Nurse to join our team full-time working Monday through Friday from 9-5 in Livonia MI. This role offers the opportunity to provide exceptional hospice care to patients in need supporting them and their families during difficult times.
Responsibilities
- Provide skilled nursing care to hospice patients in their homes
- Assess plan implement and evaluate nursing care plans according to patient needs
- Coordinate with interdisciplinary team members to ensure comprehensive care
- Educate patients and families about hospice care and symptom management
- Maintain accurate and timely documentation in compliance with regulatory standards
- Communicate effectively with patients families and healthcare providers
Requirements
- Current RN license in the state of Michigan
- Experience in hospice or home health nursing preferred
- Strong clinical assessment and communication skills
- Ability to work independently and as part of a team
- Compassionate patient-focused approach to care
