Job Summary
University Home Care Inc is seeking a compassionate and dedicated LPN Hospice to provide high-quality care to patients in the Wayne Oakland andMacomb areas. This role involves delivering skilled nursing care with empathy and professionalism ensuring comfort and support to hospice patients and their families.
Responsibilities
- Provide patient-centered nursing care in hospice settings within Wayne Oakland and Macomb counties.
- Assess and monitor patient condition managing symptoms and providing comfort care.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate patients and their families about symptom management and end-of-life care.
- Document patient progress and communicate effectively with healthcare team members.
- Ensure compliance with all regulatory and organizational standards.
Requirements
- Valid LPN license in the state of Michigan.
- Experience in hospice or palliative nursing preferred.
- Ability to work independently and with a compassionate approach.
- Strong communication and interpersonal skills.
- Reliable transportation to provide services across Wayne Oakland and Macomb counties.
