Job Summary

Position Summary

The Home Care Occupational Therapist provides skilled occupational therapy services to patients in their homes. The therapist evaluates functional abilities identifies barriers to independence and implements individualized treatment plans to improve safety and participation in daily living activities.

Essential Duties

  • Conduct occupational therapy evaluations in the home setting
  • Develop individualized treatment plans based on patient needs and goals
  • Provide interventions to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Assess home environments for safety risks and recommend modifications
  • Train patients and caregivers in adaptive techniques compensatory strategies and safe mobility
  • Recommend and instruct on use of adaptive equipment and assistive devices
  • Document evaluations progress notes and discharge summaries accurately and timely
  • Collaborate with physicians nurses physical therapists and other care team members
  • Reassess and adjust treatment plans based on patient progress
  • Educate patients and families on fall prevention and functional maintenance
  • Follow all agency policies state regulations and reimbursement requirements

Qualifications

  • Degree in Occupational Therapy from an accredited program (BS MS or OTD)
  • Current Occupational Therapist license in the state of practice
  • NBCOT certification (as required by state)
  • Valid drivers license and reliable transportation
  • Ability to travel to patient homes within assigned territory

Preferred Experience

  • Experience in home health or rehabilitation settings
  • Experience with geriatric neurological or orthopedic populations
  • Familiarity with electronic medical record (EMR) systems
  • Ability to work independently in a field-based role
  • Strong clinical reasoning communication and time management skills

Work Environment

  • Field-based position with daily travel to patient homes
  • Flexible scheduling based on patient caseload
  • Varied home environments requiring adaptability
  • Physical activity including standing bending and assisting patients during treatment
