Position: Care Manager

Reports To: Home Safe Program Manager

At Family Connections we are dedicated to providing exceptional care management services for parents children and families within supportive housing and supportive visitation service models. We are looking for a compassionate and dedicated Care Manager to join our team and make a positive impact in our community.

Your Role:

As a Care Manager you will:

  • Provide Best Practice Care Management: Offer trauma-informed and strengths-based support reassessing and setting new goals with families.
  • Supportive Housing Care Management: Conduct outreach coordinate housing intake processes and maintain communication with property managers.
  • Supportive Visitation Care Management: Facilitate supervised visitations provide comprehensive assessments and support families in nurturing parenting practices.

Transportation Responsibilities:

  • Ensure the safety and professionalism in transporting clients maintaining vehicle logs and addressing any vehicle-related issues promptly.

Qualifications:

Education:

  • Bachelors degree in Human Services or Social Work.

Experience:

  • At least two years of experience working with children and families especially those impacted by trauma and involved in child welfare.

Skills:

  • Knowledge of community resources social services and mental health systems.
  • Strong crisis intervention skills.
  • Ability to create care and visitation plans support clients and complete timely documentation.
  • Excellent communication and organizational skills.
  • Ability to work independently and as part of a multi-disciplinary team.

Salary: $42000 - $45000

Vacation & Benefits:

  • Vacation: As a full-time employee working generally 35 hours per week you will be eligible to participate in the Family Connections group benefit plans. Benefits include medical dental life insurance long term disability retirement plans and additional supplementary benefits. Yes you can elect to not enroll into these benefits. You will also be eligible to accrue vacation personal and sick days in addition to 12 paid holidays and summer wellness Fridays.

Work Environment/Physical Demands:The individual must possess the ability to perform work that requires frequent standing bending reaching squatting kneeling moving and lifting of up to or in excess of 50 pounds.A flexible working schedule with 5 days per week evening availability. Some working time is spent out of the office transporting children and/or parents to and from visits and observing visits in families homes or in the community (parks libraries restaurants jails etc.).
