Job Summary
Position Overview
Inabia is seeking a Physical Therapist to join its rehabilitation team providing therapy services to residents in a skilled nursing and post-acute care environment. The therapist will play a key role in improving patient mobility strength balance and overall quality of life.
Both experienced therapists and motivated new graduates are encouraged to apply.
Key Responsibilities
- Conduct detailed physical therapy assessments to identify mobility challenges pain levels and functional limitations.
- Develop individualized rehabilitation treatment plans designed to improve mobility strength and independence.
- Provide therapeutic interventions including exercise therapy gait training balance rehabilitation neuromuscular re-education and functional mobility training.
- Assist patients recovering from surgery stroke injury or chronic health conditions.
- Educate patients and caregivers on home exercise programs mobility safety and assistive device use.
- Track patient progress and adjust treatment plans based on clinical outcomes.
- Maintain thorough documentation in accordance with regulatory and facility requirements.
- Collaborate with the interdisciplinary team including physicians nurses occupational therapists and speech therapists.
- Meet facility productivity expectations while maintaining high-quality patient care.
Requirements
- Active Oregon Physical Therapist License (or compact eligibility)
- BLS certification required
- New graduates accepted (rate adjusted accordingly)
Schedule
- 8-hour day shifts
