Job Summary

Position Summary

The Home Care Occupational Therapist provides skilled occupational therapy services to patients in their homes. The role focuses on helping individuals improve or regain independence in daily living activities following illness injury surgery or chronic conditions. The therapist evaluates patient needs develops individualized treatment plans and works collaboratively with physicians nurses and other healthcare professionals to promote optimal functional outcomes in a home-based setting.

Key Responsibilities

  • Conduct comprehensive in-home assessments of patients physical cognitive and functional abilities
  • Develop and implement individualized occupational therapy treatment plans
  • Provide therapeutic interventions to improve activities of daily living (ADLs) mobility and safety
  • Train patients and caregivers on adaptive techniques equipment use and home modifications
  • Recommend and assist with procurement of adaptive equipment and assistive devices
  • Evaluate home environments for safety hazards and suggest modifications
  • Document evaluations progress notes and discharge summaries in accordance with agency and regulatory standards
  • Collaborate with physicians nurses physical therapists and other care team members
  • Monitor patient progress and adjust treatment plans as needed
  • Educate patients and families on condition management and injury prevention
  • Participate in care coordination and interdisciplinary team meetings

Qualifications

  • Bachelors Masters or Doctorate degree in Occupational Therapy from an accredited program
  • Current state Occupational Therapist license (required)
  • NBCOT certification (required if applicable by state)
  • Valid drivers license and reliable transportation for home visits
  • Minimum 1 year of clinical experience preferred (home health experience strongly preferred)
  • Strong knowledge of rehabilitation techniques and adaptive equipment
  • Excellent communication time management and organizational skills

Work Environment

  • Travel required to patient homes within assigned service area
  • Flexible scheduling based on patient needs
  • May involve lifting bending and assisting patients with mobility tasks
