Social Worker | Cancer Center

As an integral part of the SCH (St Cloud Hospital) Care Management System the Social Worker works with the interdisciplinary team in facilitating patient care by following the American Case Management Associations Standards of Practice: collaboration communication facilitation coordination advocacy and resource management. Is accountable for a designated patient caseload and plans care effectively to meet patient/family transitional needs monitor length of stay and promote efficient utilization of resources. Facilitates the collaborative management of patient care across the health care continuum intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.

In addition drives change by identifying areas of performance improvement and incorporating evidence-based practice. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education skills competency supports department-based goals which contribute to the success of the organization. Serves as a preceptor mentor and resource to less experienced staff. Participates in the peer review process. Abides by the National Association of Social Workers Code of Ethics.

Schedule:

• Full-time 80 hours every two weeks St. Cloud
• Day shift 8-hour shifts Monday - Friday start times between 7-8am
• Requires frequent travel to other CentraCare regional sites

Pay and Benefits:

• Starting pay begins at $30.65 per hour; exact wage determined by years of related experience

Qualifications:

• Bachelors degree or higher in Social Work or bachelors degree in Sociology with 5 years of Social Work required
• 2 years or more of clinical experience as a Social Worker
• Licensed Social Worker (LSW) Current and valid license to practice in the State of MN required
• American Care Management Association (ACMA) Case Manager-SW Certification required
• Maintains knowledge of current evidence-based best practices
• Critical thinking and organizational skills
• Ability to work collaboratively with multidisciplinary team
• Excellent oral and written communication
