Job Summary

Location: Everett WA 98201
Assignment: 12-Week Contract (Temp-to-Hire)
Schedule:
- Day Shift: 07:30 AM 04:00 PM
- MondayFriday (5x8) Every Other Weekend
Position Summary
Navitas Healthcare LLC is seeking an experienced LMSW or RN Case Manager to support acute care operations. Responsible for discharge planning care coordination and utilization management. Works closely with interdisciplinary teams to improve patient outcomes reduce length of stay and ensure safe transitions of care.
Key Responsibilities
- Coordinate discharge planning and patient transitions
- Collaborate with physicians and care teams
- Assess patients and develop care plans (Med-Surg ED LLOS)
- Monitor progress and adjust care plans as needed
- Ensure proper utilization of healthcare services
- Maintain accurate documentation in EPIC EMR
- Support compliance and quality improvement initiatives
- Float across nearby units/campuses as needed
Requirements
- 1 year acute care experience (required)
- Experience in discharge planning and care coordination
- Strong clinical judgment and communication skills
- Ability to work in fast-paced hospital settings
- Experience with Med-Surg ED or LLOS preferred
- EPIC EMR experience preferred
Education & Licensure
- MSW or RN degree required
- Active LMSW or RN license (WA or pending accepted)
- BLS certification required
