What Youll Be Doing

Youll deliver personcentred functional and environmental assessments and interventions that empower clients to maintain independence and improve quality of life. Working in clients homes youll provide specialised equipment solutions and pressure care strategies. Youll also be responsible for:

• Managing a wide variety of client presentations across multiple community aged care funding programs includingHome Care Packages Commonwealth Home Support Program Transitional Care Program and Palliative Care.

• Conducting inhome assessments and developing tailored treatment plans to help clients meet their goals. This includessimple and complex equipment prescription major and minor home modifications and client education.

• Collaborating within a supportive statewide Occupational Therapy team of 70 OTswith decades of collective experience as well as working locally with a diverse multidisciplinary team of nurses care workers and support staff.

• Tertiary qualificationsand currentAHPRA registration

• Previous experience in anAged Care or Acute setting

• Strong communication skillsand the ability to build meaningful relationships

• A currentAustralian Drivers Licenceand aroadworthy comprehensively insured vehicle

• Confidence to meet all relevant probity and compliance requirements
